| Supports community-focused health programs that promote wellness and prevent disease by connecting patients and families with local resources and reducing barriers to care. Brings hospital services and health education into the community through outreach, engagement, and education on healthy practices. Helps patients navigate follow-up care and social support services, while documenting activities and sharing feedback to improve programs. Requires a high school diploma. Typically reports to a supervisor. Works under moderate supervision. Gaining or has attained full proficiency in a specific area of discipline. Typically requires 1-3 years of related experience. |
